Registration Forms

The following forms and fees need to be turned in for each swim and dive team participant no later than April 30th.  The forms may be mailed to:

Team Fees for the 2007 Swim and Dive Season
(includes one team shirt per child)

First Child
$95 Swimming or Diving Only
$120 Both Swimming and Diving

Each Additional Child
$85 Swimming or Diving Only
$110 Both Swimming and Diving
